New Frater Paperwork (ABCD Cards and Initiation Fees):

To ensure a new frater is enrolled for membership in the fraternity it is necessary to properly process the initiate's membership paperwork. For the new frater to receive their membership package in a timely fashion this paperwork must be processed immediately upon induction. The membership package includes: 1) a letter from the Supreme Directorum congratulating the frater on their induction, 2) an AZO recognition pin, and 3) a membership card. The frater's membership certificate will be sent at a later date. Usually the certificates are signed by the officers at the next Supreme meeting after the frater's induction and either hand delivered to a chapter delegate at the Supreme meeting or mailed shortly after the meeting.

ABCD Cards:

ABCD cards are the forms used to process new member enrollment in the fraternity. It is essential that each card is filled out accurately, completely and most importantly neatly. (see Figure 1)

A Card - The A card is to be filed in your chapter's records. These records should be passed down from each Chapter Signare. They should not be destroyed. These records become useful when trying to track down Alumni fraters that have lost contact with the chapter.

Notice that each card requests the fraters home address. since upon graduation most fraters move away from home. Their parents become the only link to their new addresses.

B Card- The Director of Fraternal Affairs will be the ultimate recipient of the B card. The Chapter will not directly send the B card to the Director of Fraternal Affairs. The Supreme Signare will forward the B card to the Director of Fraternal Affairs after processing.

C Card- The C card is used by the maker of the certificates to process the membership certificates. When the certificates have been processed the maker of the certificates forwards the C card to the AZO Direct Chairman. As with the B card the Supreme Signare will forward the C card to the appropriate Supreme officer. '

D Card- The D card is used by the Supreme Excheque to generate the membership card for the new frater. The D card is also used by the Supreme Excheque to verify initiation fees sent by the subordinate chapters so that they may be accurately reflected in the fraternal financial statements. The Supreme Excheque will forward the D cards to the Director of Mailing when the financial processing is complete. The Director of Mailing will use the D card to update the computer mailing database.


ABCD Card Processing Procedures (Chapter Level)

1. No candidate may be inducted unless the ABCD cards bear the signature of the Chapter Signare.

2. Immediately after initiation, have candidates, Chapter Directorum, and Chapter Signare sign all cards.

3. Card A is to be filed in the chapter records. Cards B, C, and D are to be immediately forwarded to the Supreme Signare.

4. All cards must be filled out completely and most importantly neatly. The maker of the certificates must be able to interpret the name for it to be properly transcribed on the membership certificate.

5. The initiate's home address (where their family lives) must be used.

NOT THEIR SCHOOL ADDRESS. THE FRATERNITY WILL LOSE CONTACT WITH THE FRATER IF SCHOOL ADDRESSES ARE USED!!!!

6. A check for the initiation fees is sent with the BCD cards to the Supreme Signare. Do not send the check to the Supreme Excheque.

Initiation Fees:

1. Initiation fees are set each year at the National Convention.

2. The Initiation fee for the 2001-2002 fraternal year is $30.00 for each Undergraduate initiate and $50.00 for each Alumni initiate.

3. The check should be made out to the AZO PHARMACEUTICAL FRATERNITY.

4. The check is sent to the Supreme Signare who will forward the check to the Supreme Excheque.

Processing Membership Paperwork for Active Fraters:

It is essential that active frater membership paperwork is filled out properly and processed in a timely fashion. This process ensures that each active frater is kept on the Supreme mailing list and receives all fraternal publications and notices. Each year active fraters are required to pay an activity fee to continue their active membership in the fraternity. This activity fee is set each year at the National Convention.

The Activity fee for the 2001-2002 fraternal year is $30.00 for alumni fraters and $20.00 for undergraduate fraters. In order to efficiently process active frater membership an Activity Fee Remittance Form must be completed by the Chapter Excheque and sent to the Supreme Excheque along with a check for the appropriate activity fees. (see figure 2) When the Supreme Excheque receives the Activity Fee Remittance Form and the check for the activity fees, the Supreme Excheque generates and mails to the Chapter Excheque a membership card for each frater listed on the Activity Fee Remittance Form. The Supreme Excheque then forwards the Activity Fee Remittance form to the Director of Mailing so that the fraternity mailing list can be updated. The number of activity fees paid before each Supreme meeting will determine the number of votes a particular chapter will have at that meeting (assuming the maximum number of delegates are present). A chapter will receive 2 votes for the first 25 active fraters and 1 additional vote for each 20 additional fraters. Example 45 active fraters would yield 3 votes.

Procedures for Processing Active Frater's Membership

1. Neatly and completely fill out the Activity Fee Remittance Form. Like the ABCD cards the frater's names and addresses must be legible so that the membership cards can be filled out correctly and the mailing list can be updated with the proper addresses

2. The Frater's address must be their home address (where their family lives). Like the ABCD cards we need home addresses to keep track of graduating fraters. School addresses may also be given in addition to the home addresses. Whenever possible include the Fraters email address.

3. Send the completed Activity Fee Remittance Form along with a check for an amount that corresponds with the number of Fraters listed on the Activity Fee Remittance Form to the Supreme Excheque.

4. The Chapter Excheque will then receive a packet of membership cards which can be handed out at the next chapter meeting.

Directorums Cup

The Directorums Cup is the most important annual award in all of AZO. This award is presented to the Directorums of those chapters who showed the greatest amount of activity and progress during the year.

The Directorums Cup is a non-competitive award. More than one cup is often awarded. The scoring system is explained on page 5 and completely described in the following chapter ("Chapter Responsibilities and General Operating Procedures").

There are a possible 350 points to be achieved in the contest. A minimum of 330 points is needed to earn a cup.

The Directorum’s Cup Point Check List (figure 3) can be used by the chapters to keep track of their point totals.

DIRECTORUM’S CUP POINT SYSTEM

1. Copy of chapter minutes to Supreme Directorum, Supreme Signare, and Supreme Associate Signare - 10 points per month - maximum of 80 points per year - points awarded only when all 3 receive minutes.

2. Copy of chapter Excheque report to Supreme Excheque and Supreme Signare - 10 points per month - maximum of 80 points per year - points awarded only when both officers receive reports.

3. Chapter report to the Azoan Editor with copy to the Supreme Signare, submitted in time for publication - 20 points.

4. For each delegate present at Convention or Regional Meeting - 10 points - maximum of 20 per meeting, 40 per year. In this instance a Supreme Officer, whether elected or appointed, may be considered an accredited member of his Chapter's delegation for purposes of point crediting under this section.

5. For each issue of a chapter newspaper or newsletter 10 points - maximum of 30 points per year. Must have at least 3 issues to be eligible for a Directorum's Cup. A copy of each must also be sent to the Supreme Signare.

6. A clear financial standing with the Supreme Chapter by February 1st - 20 points.

7. Annual contribution to the Roy Scott Fund and the Ephraim G. Sless Fund - 10 points.  Send notification to the Supreme Signare.

8. Charitable fund raising or volunteer work - 25 points. Graduate chapters - the raising of money or direct contributions to a charity. Undergraduate chapters - either the raising of money or the donation of time in the form of volunteer work for charitable organizations. Mixed Chapters - either the raising of money or volunteer work.

9. Continuing education - 25 points. Graduate chapters - the sponsoring of a seminar open to all pharmacists in their area. Undergraduate chapters - either the sponsoring of a seminar or taking part in a screening of some type (blood pressure, diabetes, etc.) Mixed chapters - either the sponsoring of a seminar or taking part in a screening. The chapter must notify the Supreme Signare in writing of the charitable work or CE programs.

10. Six (6) copies of the chapter mailing list to the Supreme Signare by January 1 st - 10 points. The Signare will forward these lists to the Supreme Directorum, Supreme Excheque, Azo Direct Chairman, Director of Fraternal Affairs and Director of Mailing and keeps one himself.

11. A chapter article and chapter pictures to the Supreme Historian by June 1 - 10 points. Also a copy must be sent to the Supreme Signare.

12. List of graduating fraters (including home addresses) sent to the Supreme Signare at the completion of the school year. No points but mandatory to receive the Directorum's Cup.

Total number of points available is 350. Any chapter receiving a total of 300 points or more would receive a small cup (approximately 6 inches tall). The Chapter receiving the greatest number of points would receive a large cup (approximately 12 inches tall).

1. A copy of all information required for any points shall be submitted by the chapter to the Supreme Signare in addition to any other involved officer
2. Any reference to a specific date or period of time specified within the point-scoring
system shall be strictly adhered to for the issuance of points.
3. A chapter must respond to every request from the Supreme Signare for a vote on a
constitutional amendment to be eligible for a cup.
4. All cups shall list the number of consecutive years a chapter has won a cup.
5. A chapter must include the Supreme line officers, Historian and Azoan Editor on its mailing list to be eligible for a cup.

Chapter Responsibilities and Operating Procedures:

Most of the day to day procedures required of each subordinate chapter are used as criteria for scoring the Directorums Cup. To simplify this manual the cup requirements and explanations of the subordinate chapter's responsibilities will be discussed together.

Chapter Meeting Minutes:

1. Minutes for each chapter meeting should be transcribed by the Chapter Signare. Supreme supplies each chapter with a special minutes form (see Figure 4). The Supreme Signare supplies these forms.

2. Copies of the meeting Minutes must be sent to the following Supreme Offlcers.
A) Supreme Directorum
B) Supreme Signare
C) Supreme Associate Signare
(A,B,C, required for Directorums Cup Points)
D) Undergraduate Representative
E) Supreme Council Member
F) Undergraduate Advisor

3. Copies of the meeting minutes must be sent within 30 days of the date the meeting was held.

4. Meetings must be held at least once a month for 8 months. The months may vary depending on school schedules.

5. A maximum of 80 points is available (10 points for each month). Points are awarded only when all three Supreme Officers listed above receive the minutes.

Chapter Excheque Reports:

1. Chapter Excheque reports must be sent monthly to the following Supreme Officers:
A) Supreme Excheque
B) Supreme Signare
C) Supreme Directorum
D) Undergraduate Advisor

2. Special Excheque report forms are supplied by Supreme. The Supreme Signare supplies these forms (see Figure 5).

3. The Excheque reports must be sent each month for 8 months. The months will be determined by your school schedule.

4. A maximum of 80 points will be awarded (10 points for each month). Points will be awarded when all three Supreme officers receive the Excheque reports.

Azoan Report

Each year the chapter is required to send a report to the Azoan editor for inclusion in the Azoan. This report outlines the good things that happened during the fraternal year at your chapter. Usually your Chapter Historian is the author of this report.

The report is valued at 20 points for the Directorums Cup points

Chapter Newspaper

The chapter newspapers are used not only for scoring for the Directorums cup but are also judged in our yearly Newspaper Cup Contest. The Newspaper Cup Contest will be discussed later. The chapter newspaper is the voice of the chapter and as such is an important communication vehicle not only to keep the Fraters in your chapter up-to-date on what is happening in the fraternity, it also helps the Supreme officers stay informed of chapter activities. The Supreme Associate Signare will many times take information from chapter newspapers and include it in the Supreme Newsletter.  To be eligible for the Directorums Cup your chapter must submit at least 3 issues of the newspaper.

Ten points are awarded for each issue with a maximum of 30 points to be awarded. To receive the maximum number of points you must submit 3 issues of your newspaper. Issues must be published between October 15th and May 15th.

Chapter Tax

Chapter tax is a fee charged to each chapter in the fraternity. The alumni and mixed chapter tax is more than the undergraduate chapter tax. The chapter tax is set each fraternal year at the National Convention.

T'he chapter tax for the 2002-2003 Fraternal year is $65.00 for alumni and $35.00 for undergraduate chapters.

Good Financial Standing:

Each chapter must be in good financial standing with Supreme by February 15th of each fraternal year. This means that the chapter tax and activity fees must be paid by this time.

Attending Supreme Meetings:

Your chapter will receive points for the Directorums Cup for attending Supreme meetings. 10 Points will be awarded for each delegate with a maximum of 20 points per meeting. A maximum total of 40 points will be awarded for attendance at Supreme meetings.

Leadership Conference:

This conference is held at the fall regional for the education of the undergraduate and alumni fraters. Since we will only have a winter regional this year, a manual explaining the operations of the fraternity at the local and Supreme levels will be mailed to all chapter Directorums in the fall.

Contributions to the Roy Scott and Ephriam G. Sless Funds:

Each year subordinate chapters are requested to contribute to the Roy Scott and Ephriam G Sless funds. 10 points are awarded for contributions to each fund. A maximum of 20 points will be awarded.

Ephraim G. Sless Fund

The scholarships given out by Supreme to AZO undergraduates are funded by the Sless fund.            
Only the interest received each year is used for the scholarships. Principle is not touched. Obviously the more money raised for this fund the more money will be available for scholarships.

Roy Scott Fund

The Roy Scott fund is the fraternity's charitable foundation. At one time continuing education seminars were funded through the Roy Scott fund. This practice has recently been discontinued.

Charitable Work

As healthcare professionals we are responsible not only to our patients but to the community as well. In this spirit AZO undergraduates are encouraged to become involved with local and civic charities as well as the AZO national project. Charitable work may include anything from raising funds for a local charity to helping senior citizens in the neighborhood to take out the trash or shovel snow from their sidewalks.

Charitable work is worth 25 points in the Directorum Cup scoring.

Continuing Education:

For alumni chapters AZO encourages the running of accredited continuing education programs. AZO undergraduates can also get involved educating the public through activities such as blood pressure screenings, speaking to school groups on topics such as diabetes or drug abuse.

25 points are awarded for continuing education

Chapter Mailing List:

By January 1st of each year your chapter is required to send six(6) copies of your chapter mailing list to the Supreme Signare.

10 points are awarded for the mailing list in the Directorums Cup scoring.

List of Graduating Fraters:

At the end of each school year it is the responsibility of each undergraduate chapter to send a list of graduating Fraters to the Supreme Signare including home addresses. No points are given for this list but a Directorums Cup cannot be awarded unless this list has been received by the Supreme Signare.

Historian Article:

At the end of each school year it is the responsibility of the chapters to send an article detailing the activities of the chapter during the fraternal year and a chapter photograph. Usually this is authored by the chapter Historian.

Scholarships:

Scholarship applications (figure 6) are sent out at the beginning of the year usually in the first    newsletter. Additional copies can be obtained from the Effie Sless Fund Chairman. The deadline for submission is April 30th. Usually three to four scholarships are given annually. The number and amount depend on available funds form the Ephriam G. Sless Fund.

Undergraduate Pledge Seminar Reimbursement
(Reimbursement for Rush Parties):

Each fraternal year Supreme will reimburse undergraduate chapters up to $350.00 for pledge periods, maximum of 2 per school year. Send receipts for expenses incurred at your rushes to the Supreme Excheque along with a list of prospective members and completed initiation forms if applicable.

The Sidney R. Rome Newspaper Cup Contest:

This contest is administered by the Supreme Sub-Directorum. Three Judges (usually from outside the Fraternity) are chosen by the Supreme Sub-Directorum to judge the contest. Newspapers are judged on content not the quality or method of printing. The deadline for this contest is May 15th.

The judging criteria is as follows.
    1. Professional articles.
    2. General pharmaceutical topics.
   
3. Local chapter news.
    4. Editorial content.

Don't be afraid to have some "fun " articles. Remember above all your newspaper is for your chapter so have fun with it.

Newspaper circulation:

1. Everyone in your chapter.
2. Every Supreme officer.
3. The Officers in your city's alumni chapter (if you have one).
4. Five (5) copies to the Supreme Sub-Directorum.

Cups are awarded in the following categories:

1. Alumni and Mixed Chapters.
2. Undergraduate Chapters.

Only three newspapers are required for the newspaper cup contest.

Undergraduate and Graduate Fraternity Awards

Undergraduate award

This award is presented by the Supreme Directorum. It is the highest honor an undergraduate can receive in AZO. Selection criteria include fraternal activity, leadership, scholarship and citizenship. Each undergraduate chapter nominates a frater from their chapter to receive the award. This in itself is a great honor. Some undergraduate chapters present their nominee with a plaque to commemorate this honor. Candidates for the award are required to submit to the awards committee (the Supreme Sub-Directorum is the chairman) a biographical sketch, a transcript of the grades and letters of recommendations by June 1st. The awards committee will select and announce award recipient (s) at the Summer Convention and the award will be presented at the Winter Regional.

The Simon I. Sless Award

This award is presented to an undergraduate frater for outstanding service to the fraternity on a local level. Recipients for this award are selected by the local chapter. The Supreme Signare is then notified that the award is being presented. The Supreme Signare will order the award and notify the chapter when it is received.

The Ephriam G. Sless Award

This award is presented to a graduate frater for outstanding service to the fraternity on a chapter level over a long period of years. The Supreme Signare is notified of the recipient and will order the award.

Order of the Double Star

This award is presented to those fraters who are serving as president of a local, county or state pharmaceutical association; office in a national pharmaceutical association; president of a college of pharmacy alumni association; member of a state board of pharmacy; or a faculty member of a college of pharmacy.

Fraternity Jewelry

Fraternity jewelry is ordered through the Supreme Signare - Jewelry prices are set at the beginning of the fraternal year. The Supreme Signare will notify each chapter of jewelry prices at the beginning of the year. Usually delivery takes approximately 8 weeks but could take longer depending on the order. 

Shipping and handling is $4.95 per order. State sales tax is applicable for shipments in New England states only (ME, VT, MA, NH, CT, RI). Engraving is an additional $3.50 for each piece of engraved jewelry.

Jewelry is not 14k gold, but an alloy called Goldgloss.

Goldgloss is Legacy Jewelry’s trade name for a finely polished gold electroplate finish. The rubies are synthetic stones.

Pledge pins should be recycled rather than given to each pledge.

No jewelry will be ordered without pre-payment.

Pledging

This manual will not attempt to explain pledging procedures in detail. But several points which seem to come up each year can be touched upon.

Pledge Manuals

Pledge manuals should be recycled through many pledge classes. Only when the books are excessively worn should new ones be requested. Remember new manuals are available through the Supreme Signare. The pledge manual is an important part of pledging. No pledge should become a Frater until he or she knows this book inside and out.

Pledge Pins

Pledge Pins like the pledge manuals should be recycled through many pledge classes. New Pledge Pins can be ordered through the Supreme Signare. Each pledge is required to have a pledge pin on his or her person at all times during the pledge period. It is important that the pledge understands the symbolism of the three sides of the triangle.

Pledge Books ( not required)

Many undergraduate chapters require their pledges to carry around a small pocket sized note pad. This pad is used to get signatures of the Fraters and to obtain personal information about the Fraters such as home town, hobbies, etc. This is an excellent vehicle to foster friendship between the prospective member and the active Fraters.

Physical Hazing

Physical hazing of a pledge is strictly forbidden by Alpha Zeta Omega. No exceptions!!!!!

Procedure Calendar

September (or beginning of school year)
    1. Send 6 copies of mailing list to Supreme Signare.
    2. Begin collecting activity fees from active fraters.
    3
. Find out when and where Winter Regional will be held and publicize to chapter.    
    4. Try to obtain a central mailing location at school that can be used year after year. This will
make it easier for Supreme to communicate with the undergraduate chapters on a regular basis.
    5. Elect or appoint a Supreme Council Member from your chapter and notify the Supreme Signare.

October
    1. Send in minutes from September chapter meeting to Supreme officers.
    2. Send in September’s Excheque report to Supreme officers.
    3. Begin thinking about publishing first issue of newspaper.
    4. Begin making travel plans for the Winter Regional.
    5. Select your chapters candidate for the Supreme Undergraduate Award
    6. Start to gather a biographical sketch, transcript of grades and letters of recommendation for your undergraduate award candidate. Send them to the Supreme Sub-Directorum by December 1st!!!
    7. Do you have an agenda for your chapter’s charitable work, national project, continuing education, screenings, etc.

November

    1. Send in minutes from October chapter meeting to Supreme officers.
    2. Send in October’s Excheque report to Supreme officers.
    3. By now your first issue of your chapter newspaper should be published. Send 5 copies to the Supreme Sub-Directorum.
    4. Begin to prepare your chapter report for presentation at the Winter Regional. The report should be sent to the Supreme Signare at least 10 days before the Regional.

December

    1. Send in minutes for November chapter meeting to Supreme officers.
    2. Send in November’s Excheque report to Supreme officers.
    3. Another newspaper should be published soon. Send in 5 copies to the Supreme Sub-Directorum.
    4. Have you sent in your activity fees and chapter tax yet?
    5. Have you sent in your 6 copies of the chapter mailing list ? Deadline is January lst.

    1. Send in minutes for December chapter meeting to Supreme Officers.
    2. Send in December's Excheque report to Supreme officers.
    3. Your second newsletter should be published by now!! Send 5 copies to the Supreme Sub-Directorum.
    4. Are you in good financial standing? The deadline is February 15th!!
    5. Have you chosen a recipient for the the Simon Sless Award (undergraduate chapters) or the Ephriam Sless Award (alumni chapters). Send the name of the recipient and the date of presentation to the Supreme Signare.
    6. Scholarship applications due by April 1st to the Effie Sless fund chairman.

February

    1. Send in minutes for January chapter meeting to Supreme Officers.
    2. Send in January's Excheque report to Supreme officers.
    3. Don't forget you still need at least one more newspaper published before the end of the school year. Send 5 copies of each to the Supreme Sub-Directorum.
    4. The deadline for good financial standing is February 15th. Are your activity fees and chapter tax paid?
   
5. Contributions to the Ephriam G. Sless and Roy Scott funds must be made soon.
    6. Make sure charity and continuing education activities are in the chapter minutes so that you get credit for the Directorums cup.
    7. Start thinking about Azoan report. Send a chapter picture to the editor!
    8. Scholarship applications due by the 15th of March.
    9. Should start to prepare Historian report for the year
    10. Make sure that your chapter delegate to the Winter Regional gives a full report at your chapter meeting.

March

    4. Don't forget to send in your Spring Rush receipts to the Supreme Excheque for reimbursement. Supreme will cover up to $200 per year. $100 per semester.
   
5. Review Directorums Cup Checklist. Check scoring list in Supreme Newsletter. If there are anydiscrepancies contact the Supreme Signare immediately.
    6. Azoan report may be due!! Did you send in your chapter picture.?
    7. Scholarship applications due by the 1st of April..
    8. Find out when and where the Summer Convention will be held and announce to chapter.

April

            D) At least 3 newspapers.

The Vial of Life Program

What: ‘Vial of Life’ - Life saving information in your home in case of crisis or emergency.

For Whom: Individuals who live alone, Senior Citizens, young mothers with medical problems, or anyone who wishes to provide for care in case of fire, sudden illness or other emergency.

Where: A small plastic vial taped above the upper right hand shelf of your refrigerator - to contain necessary information regarding the residents to be available and easily found by the emergency squad, police, firemen, etc. if the need arises.

The Program: ‘Vial of Life" was first initiated by Deputy Sheriff Richard Marsh of Monroe County, Michigan. The program is sponsored locally by the Alpha Zeta Omega Pharmacy Fraternity. Instructions: Just fill out the enclosed form, tuck it into your vial, tape it in your refrigerator, and place the sticker on the outside of the refrigerator in the upper right hand corner. Then rest assured that you will provide all of the information needed to take care of you in an emergency. -------
