Dr. Jeffrey M. Levy Scholarship
The New York Alumni Chapter of Alpha Zeta Omega is proud to sponsor the second Dr. Jeffrey M. Levy Scholarship. Jeff was a dedicated Frater, pharmacist and medical doctor. Even though medicine was his calling, he never forgot his pharmacy roots and remained active in both New York Alumni and the Supreme Chapter. To honor Jeff’s legacy, New York will be awarding a $750 scholarship to a pharmacy student who has shown dedication to his or her chapter and to Supreme. Sless Memorial Fund Scholarship In order to be eligible to receive scholarship assistance, the applicant and his/her chapter must be in good standing with the Supreme Chapter of the Alpha Zeta Omega Pharmaceutical Fraternity. The applicant shall have the responsibility of confirming the status of both themselves and his/her chapter. Donations into the Ephraim G. Sless scholarship fund and profits from investments made in the Ephraim G. Sless Memorial Fund are distributed back to undergraduate members through the "Founding Fathers Scholarship Fund". Awards
Achievement Award
An outstanding award that is presented to either a Fraternity member or non-fraternity person for outstanding contributions in the field of pharmacy and allied sciences, or awarded to a non-fraternity person for outstanding contributions to Alpha Zeta Omega. The Supreme Awards Committee is responsible for selecting an Achievement Award Recipient. For a full list of previous Achievement Award recipients, click here. Meritorious Award
An outstanding award that is given to an Alpha Zeta Omega Frater who has contributed in outstanding fashion, superior work and accomplishments for the Fraternity over the period of a number of years. This award is presented to those fraters who are serving as president of a local, county or state pharmaceutical association; office in a national pharmaceutical association; president of a college of pharmacy alumni association; member of a state board of pharmacy; or a faculty member of a college of pharmacy. Notify Supreme Signare when a Frater is eligible to receive this prestigious award given at a function or meeting of that organization at which the Frater is being honored. |
Jay L. Pollock Supreme Undergraduate Award
This award is presented by the Supreme Directorum. It is the highest honor an undergraduate can receive in AZO. Selection criteria include fraternal activity, leadership, scholarship and citizenship. Each undergraduate chapter may nominate one (1) frater from their chapter to receive the award. This in itself is a great honor. Some undergraduate chapters present their nominee with a plaque to commemorate this honor. Candidates for the award are required to submit to the awards committee (the Supreme First Sub-Directorum is the chairman) a statement of intent, an unofficial transcript from their school, one letter of recommendation, and their CV/resume by June 1st. The awards committee will select and announce award recipient(s) at the Summer Convention and the award will be presented at the Winter Convention. Click here for a list of previous Jay L. The Simon I. Sless Award
This award is presented to one (1) undergraduate frater per Chapter for outstanding service to the fraternity on a local level. Recipients for this award are selected by their local chapter. The award recognizes by name AZO's first pledge, Simon Sless, 1925-1926 Supreme Directorum.
After a Chapter has selected a recipient, the Chapter must notify the Supreme Signare so that the award may be ordered. Submissions must be sent to the Supreme Signare no later than March 1st.
This award is presented to one (1) undergraduate frater per Chapter for outstanding service to the fraternity on a local level. Recipients for this award are selected by their local chapter. The award recognizes by name AZO's first pledge, Simon Sless, 1925-1926 Supreme Directorum.

This award is presented to one (1) graduate frater for outstanding service to the fraternity on a chapter level over an extended period of time. No more than one award shall be granted to any one chapter during any one fraternal year. The award recognizes by name AZO's founder, Ephraim G. Sless, 1923-1924 Supreme Directorum.
If Chapter has selected a recipient, the Chapter must notify the Supreme Signare so that the award may be ordered. Submissions must be sent to the Supreme Signare no later than March 1st.
